Yes, I hereby wish to confirm that I would like to apply for LayBy / Instalment sale. 

I hereby authorise Table Charm Direct or its authorised service provider to commence a debit order withdrawal from my account on the 'debit date' selected above, and monthly thereafter, for the amount applicable to the agreement I have entered into. I understand that the debit order will be processed on the date selected. If for any reason it is not honoured, two withdrawal attempts will be made in the following month. Should the second attempt also be dishonoured, the agreement may lapse.

I acknowledge that this signed consent form must reach the offices of Table Charm Direct at least 10 working days prior to the selected deduction date. Failure to meet this deadline will result in the first deduction only being processed in the following calendar month.

In the event that the payment date falls on a Sunday or a recognised South African public holiday, the payment date will automatically be adjusted to the preceding business day. I further understand and agree that if there is any inflation-related adjustment to the applicable payment, the adjusted amount may be deducted from my account.

I acknowledge that I may cancel this Authority and Mandate by providing Table Charm Direct with at least 20 ordinary working days’ notice in writing. However, I understand that such cancellation does not cancel the agreement itself. I shall not be entitled to a refund of any amounts deducted while this Authority was in force, provided such amounts were legally owing.

I confirm that payment instructions issued under this Authority will be treated as if I issued them personally as the account holder. I further acknowledge that this Authority may be ceded or assigned to a third party if the agreement is similarly ceded or assigned, but in the absence of such an assignment, this Authority and Mandate may not be ceded or assigned.

The User Abbreviated Name as registered with the bank will reflect as follows on your bank account: Table Charm followed by your policy or membership number.
